In the wound manager you can change the wound stage under change in wound type and selecting "yes". Once a wound is staged it can progress to a higher stage, but can never be down-staged. Under the wound type you will see the options for changing the stage of that particular wound. Please reach out with further questions.

Thank you for posting your question in the User Community. I would like to include a helpful resource from our Help Center that will provide you with a wealth of helpful information pertaining to the functionality of the Wound Manager.
To answer your question, if a clinician needs to change the stage of a pressure ulcer, the Change in Wound Type buttons enable users to restage wounds and document tissue type changes in non-pressure ulcers on subsequent visits. However, if the staging of a pressure ulcer needs to be changed for a completed/past visit, please see the following excerpt from the resource I will include to follow:
"Since the Wound Manager is an automated system, visit management is extremely important to maintain data integrity. Late documentation disrupts coordination of patient care and data in subsequent visits. Once a subsequent visit for a patient has started, the following actions cannot be performed in late documentation: Adding New Wounds, Deactivating Wounds, Change to Tissue Type (Restaging), Reactivating Wounds... Any late entries must be added as an addendum to capture desired documentation. As a reminder, wound documentation can be completed/signed by the user before completing the clinical note or comprehensive assessments (i.e., OASIS)."
