Can I get some feedback on how people/organizations are utilizing the addendum/PATIENT NOTIFICATION OF HOSPICE NON-COVERED ITEMS, SERVICES, AND DRUGS upon admission? There is some confusion about what exactly needs to be documented and signed/selected when it comes to the addendum. How do organizations note whether the addendum was discussed and offered and either accepted or declined by the patient/patient representative? When the following is selected in the summary portion of the Initial/Comprehensive assessment, is it just assumed that the addendum was offered and declined? Thank you for your question regarding the Election Addendum.

When addressing the discussion regarding the election addendum, that is located in the Admission Visit on the Summary tab when addressing the “First Visit Interventions” The selection for Hospice Coverage and Financial Responsibility addresses the right to request the Non Covered Items. It is also commonly addressed on the Hospice Election form that it has been requested.

The management of the Addendum is completed under the Patient tab in the Non-Covered Items. You are able to generate the addendum and manage the signature/refusal of signature and upload the completed document when it has been signed or refused. You may also decide to use a Tag for election addendum to ensure that there is follow up on getting the document completed in the timeframe compliant to CMS guidelines. Another option is to use the "Admission to Hospice" Plan of Care item and select "other" under the Goals to add Election Addendum follow up to ensure it is being addressed within a 5 day goal length.
