Hello, Can you create a DC summary on axxess for hospice patients, or Do you have to upload a document?

Thank you for using the User Community. There is not a task titled "Discharge Summary" but an agency could create a template to be utilized to capture anything else the agency does as part of their discharge process. The clinician could then pull up the Discharge Summary Template (should you title it to that effect) in their Discharge Visit/Unattended Discharge Note/LPN-LVN Discharge Visit.
