I am aware of intent/coding instructions for assessment of the patient. My question is, the patient was not assessed due to a non-visit. I have not been able to find specific guidance on how to address cognitive questions. Would it be more appropriate to use "information not available" or whether the answers would be "no" since patient could not answer question (assessment not conducted).
