Is any one having issues when using PT Assessment SOC? Goals upon discharge are going to SN goals. The chart is a PT case only and no nurse assigned to the patient episode.

On the OASIS (PT) SOC there is a great option to complete the Tinetti POMA assessment tool and it is then imbedded into ... same item into subsequent assessments?
