Does anyone else have issues when therapy turns in an eval, the therapy POC sits in saved mode? Is it required that the therapist sign both the eval and the therapy POC? 

The nurses sign the OASIS and the POC is signed automatically.

I am really looking for a NEW innovative way to perform medication reconciliation for nurses. So much time is spent transcribing and looking up medications, is there any way that AXXESS can develop or test a better medication reconciliation process? Like scanning Barcode on a medication bottle using the AXXESS app that transfers the information straight to the med list? 

Attempting to add new wounds and order from SN note, received message: There are no unassociated wound orders for this patient. To add a new wound, a wound order must first be created through a physician order. Click the Physician Order button to create a wound order and return here to add the wound.....I clicked on the phys,. order to create and I continue to get the same message. Videos did not help... frustrated...
