unable to print discharge summary document? all of my other orders will open and print but not this one in particular. is this a known issue for anyone else?

unable to create incident log. cannot choose patient name. everything is grayed out.
User does not have permission to bypass QA and also under company information the box is not checked for orders to bypass QA, however orders are still bypassing.
For my Medicare and Medicare Advtg claims, I have the change rule check marked for the G0154 code within the payer setup, however my claims are not showing up any G-codes at all - procedure codes are blank. I had to manually remove the G0154 and type in the correct code to get my codes to pull over onto the claim.
From the Billing center, when i click on the patients name to go to their chart - it pulls up discharge patients and the list is blank, so i have to manually go to "active" patients and scroll to the patients name. instead of taking me straight to the chart like it used to. Is anyone else experiencing this?
Is there a quicker way to print patient charts rather than choosing each document to print one by one?
