OASIS-E DISCHARGE
Anyone know why there is no option to schedule a OASIS-E Discharge?

Red Sticky Note
Can a red sticky note be deleted? One of our nurses put one on an OASIS she has no attachment to and it does not flow with our diagnoses, etc. I've never had this happen before.

Moving a visit
We have a visit that shows up on schedule deviations and three of us have tried (and failed) to move it to new episode. We've tried changing dates, moving to new episode within details but nothing is working. We continuously get an error that says "not within episode range". The new cert period has been created so it does has a new episode to move to. The only thing I can think of is that it is in "saved" mode.
Anyone else encounter this issue?

PLAN OF CARE
SN did a recert and then several days into the new cert period did a discharge. Now, when I try to go over the recert, the plan of care (purple) section is completely blank. I'm assuming SN did goals met when he did the discharge. Is there a way to get the Plan of Care section to come back so that I don't have to re-do the entire Plan of Care section for the recert 485? PLEASE HELP!
I have a paraplegic patient and the OASIS Neuro section has paraplegia and quadriplegia greyed out. Does anyone know what drives those items to be greyed out? I don't know which questions the nurse answered that would make those be greyed out. Help!

I know this is a somewhat ridiculous question having been using Axxess for several years but
When in the Functional Status of the Recert Oasis, where do the answers pull from? I've got grayed out upper body dressing but lower body dressing I can mark the 1st option. Bathing is the same way it has 0, 1 grayed out so I can only choose 2 or worse. This patient is blind, but quite functional; however, I cannot get to the correct M00s because they are grayed out. I'm wondering what I marked that would gray out those M00s. Any help?

How do I get a patient removed from an SN patient list on the App for Home Health if it's no longer his patient? Nurse is getting frustrated because there are patients that he's NOT supposed to see clogging up his true patient list. He also has duplicate patients listed?
