Why do medications that have been discontinued on the med profile still carry through to the auto-generated DC summary?
Also, why do medications that were entered as "new" by the nurse at the SOC visit NOT show up on the auto-generated DC summary even though the med was not discontinued by any of the providers? The meds show as discontinued as of the date the nurse discharged the patient.
When a user has the role of "office staff" assigned should that allow them to view all patients? Active, discharged, pending, etc?
Is it possible to create custom reports in Axxess that would aid in the pulling the information for data fields required for HHCAHPS surveys? Are custom reports even possible? Or does anyone use a particular report to pull this data when using an outside vendor for HHCAHPS?
The Oasis E DC (PT) scheduled do not show a section for Reconciled Medication List with questions A2123 and A2124. This was present on Oasis E DC notes done prior to 1/13/23 but does not show up on the same note type scheduled for after 1/16/23. Is this a software glitch or that section is no longer on the Oasis E discharge (PT)? Any other reason why that section would not show up on discharges to be completed?
We are saving PDF documents to Axxess chart but when we try to open that document from Axxess it does not download as a pdf but just a "file" that we then have to choose adobe to open. Is there a setting to make sure all documents in axxess are always pdfs when pulled from patient documents? These files are PDFs on our hard drives and cloud storage and open as PDFs correctly.
Another issue today unfortunately, one of our PTs does not have the option to "check for errors" when signing off his PT evals and re-evals. Because of that a PT POC is not being generated either. The SN POC has already be approved and sent to MD so PT is not pulling into that POC. Any other settings or things to check to correct that?
Am I crazy or didn't there used to be a column to the far left in the Schedule Center that was not just for Authorizations but showed a green check if the scheduled visits and order frequencies matched for each patient. Is that not a feature in axxess any more? I submitted the ticket but they only spoke of Auths. This used to be present even on our straight Medicare patients who have never required auth and an auth is not marked as needed in the payer set up.
If CAHPS is an option on my dashboard and we have the option to choose to send patients CAHPS surveys or not when admitting are we set to begin using this feature? Do I need to select an authorized vendor? Is this feature included in our monthly price already? We are a new agency that will just now begin utilizing this feature. Any insight is helpful. Thank you.
I am entering UTNs and affirmed decisions, marking saved and received. Moves into PCR history as non-affirmed and does not save the UTN/affirmed response. Is this a system issue? Was working fine just a few days ago.
